STEP TWO: Prepare your Submission

How to Prepare and Submit Your Application for IRB Review

1. Please see the IRB Forms & Templates page to download the MOST current IRB Application, templates, and guidance documents.
    
2. Make sure your CITI training is complete and current. CITI certification is valid for three years. After that, CITI will require a "refresher course" to maintain training certification. Login to your account at citiprogram.org to check your certification status.
    
3. Familiarize yourself with Informed Consent.
    
4. Complete the IRB Application for Human Subject Research by carefully following all the form instructions.
    
5. Include all supporting documentation with your application form:

• Recruitment documents (emails, flyers, social media posts, etc.)
• Consent/assent documents
• Study instruments (surveys, interview questions, tests, etc.)
• Letters of authorization (if needed)
